Telehealth Informed Consent

Offline Telehealth LLC

Effective Date: June 16, 2026

What is Telehealth?

Telehealth involves the delivery of healthcare services using electronic communications. Offline Telehealth uses an asynchronous consultation model, meaning you submit your health information online and a licensed provider reviews your case and responds without a real-time video or phone appointment. The practice of telehealth is governed by the laws of the state where you are located at the time services are delivered.

Technology and Security

Your consultation is conducted through our platform, which is powered by Rimo.co. All data transmitted during your consultation is encrypted in transit and at rest. Our platform uses secure, password-protected access and is designed to meet HIPAA security standards. No session recording occurs. While we implement industry-standard security measures, no electronic transmission is entirely without risk. In the unlikely event of a technical failure during your consultation, you may contact us at support@offlinetelehealth.com to continue your care.

What You Are Consenting To

By using Offline Telehealth’s services, you consent to the following:

Asynchronous Consultation Your consultation is conducted asynchronously. You will complete an intake questionnaire and a licensed provider will review your responses and make an independent clinical determination. The provider will disclose their identity, licensure, title, and applicable specialty as part of the consultation process. There is no real-time video or phone component unless specifically arranged.

Provider Independence and Licensure All treatment decisions are made solely at the discretion of the licensed provider reviewing your case. Providers are licensed in the state where you are located at the time services are delivered. Offline Telehealth does not guarantee that a prescription will be issued. The provider may determine that treatment is not appropriate for you, or that in-person care is required, based on the information you provide.

Limitations of Telehealth You understand that telehealth has potential risks and limitations, including but not limited to:

  • The inability of the provider to conduct a physical examination
  • Delays in evaluation or treatment due to technical failures
  • Incomplete transmission of medical information
  • Potential information loss due to technical or communications failures
  • The possibility that the provider may refer you to in-person care if telehealth is not clinically appropriate for your situation
  • The provider’s clinical judgment may be limited by the information available through an asynchronous format

Compounded Medications Treatments prescribed through Offline Telehealth may include compounded medications. Compounded medications are not FDA-approved finished drug products. They are prepared by a licensed compounding pharmacy based on a valid, patient-specific prescription from your provider. The FDA does not review or approve compounded medications for safety or effectiveness. Individual results may vary.

Alternative Treatment Options In-person care and FDA-approved prescription medications are alternatives to the services offered through this platform. Your provider will discuss these alternatives with you as part of the consultation process if appropriate.

Voluntary Participation Your participation in telehealth services is entirely voluntary. You have the right to refuse or withdraw consent at any time without affecting your right to seek care through other means.

Not for Emergencies Telehealth services are not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

No Guarantee of Results Individual results from any prescribed treatment may vary. Offline Telehealth makes no guarantees regarding treatment outcomes.
